Online Registration Instructions for
CVU High School
 
STEP 1:   Follow this link to FormReLeaf and click on “SIGN UP” at the bottom left corner.

• Register as an individual and create an account.

STEP 2: Log in with the email and password just created.

STEP 3: Under “Sections” at the bottom of the page:

1. Select "ALL STUDENTS - Emergency Medical Information - Health Office" and, if your child anticipates playing a sport during any season, select the appropriate season and sport at this time.
2. If this is your first time registering your student using Form ReLeaf,  you will need to  fill in all fields. (Any question with a red star is required in order to complete registration.)  If you have previously registered your child in Form ReLeaf, use the Autofill feature.  Please make sure to review the information we have and update information as needed. Check to be sure the student "Grade" is current.
3. When finished, click “Submit” located in lower right hand corner.
4. When you have completed the online registration process, you will receive a confirmation email.

If you are signing up to play a sport, after completing online registration packet:

• Unless we have a current CVU Well Exam form on file, print the form and bring it to your physician.
• Once your physician has completed the physical exam documentation, return the completed form to the Main Office prior to try-outs.
